It gives Courtier and Guest Opinion -10, Enemy Hostile Scheme Phase Length 20 days slower per Scheme Phase. The script puts it on a character. One place in the game's script adds it: Visions of Darkness (event trait_specific_ongoing.1012). Patch 1.20.1.2, from game/common/modifiers/00_health_modifiers.txt.
