It gives Parochial Vassal Opinion -15, Courtly Vassal Opinion -15. The script puts it on a character. One place in the game's script adds it: Keen as Blade's Edge (event ep2_accolade_events.0200). Patch 1.20.1.2, from game/common/modifiers/04_ep2_modifiers.txt.
